Your first assignment for this week is to be written in a SOAP Note format (NO NARRATIVES). FOLLOW THE TEMPLATE: There is a template/sample for you to follow posted in your announcements at the beginning of the semester. You will need to put in the missing information in the note (some may be made up ie meds, hx, parts of the ROS and PE). I’m looking to make sure you know what information to include. In the Assessment/Plan, you will document your differential diagnoses as per the assignment. I will comment on your notes and if needed, send you an email if I need to help you with them more.

Assignment 1:

The Lab Assignment

·Choose one skin condition graphic (identify by number in your Chief Complaint) to document your assignment in the SOAP (Subjective, Objective, Assessment, and Plan) note format rather than the traditional narrative style. Refer to Chapter 2 of the Sullivan text and the Comprehensive SOAP Template in this week’s Learning Resources for guidance. Remember that not all comprehensive SOAP data are included in every patient case.

·Use clinical terminologies to explain the physical characteristics featured in the graphic. Formulate a differential diagnosis of three to five possible conditions for the skin graphic that you chose. Determine which is most likely to be the correct diagnosis and explain your reasoning using at least three different references, one reference from current evidence-based literature from your search and two different references from this week’s Learning Resources.

By Day 7 of Week 4

Assignment 2:

This week, the Health History Assessment Shadow Health assignment is due by Day 7. There is a link provided for you to submit your lab pass. I have provided many announcements, links, and tips to help you through this process. Please review all announcements, shadow health information and read and follow the RUBRIC and use TEMPLATE provided- failure to so will result in failure of the assignment. There are specific instructions in Module 3, Week 4 on where and how to submit each assignment.

Be mindful that you may have as many attempts to complete the Shadow Health Assignment to increase your DCE score until the last day of the assignment (day 7). Any submissions after that will not count towards your grade. You will need to submit your documentation separately, to the appropriate dropbox. Please review the grading rubric for this assignment. Once you complete your assignment in Shadow Health, you will need to download your lab pass and uploaded to the appropriate drop box. There are 2 parts that you will be graded on.

Please be sure that you are using the Shadow Health Template to document your ALL Shadow Health SOAP notes. Use the Comprehensive SOAP note template as a guide to help you gather all the information that is needed to document on the Shadow Health Template.

Please note that failure to follow directions concerning documentation using the Shadow Health Template will result in a poor score. Week 4 Shadow Health assignment must be passed in order to continue in the course.
